Support at Home Care Partner / Case Manager

Dolphin Aged Care provides in-home aged care support to older people in our community. This may include Aboriginal and Torres Strait Islander clients and non-Aboriginal clients.

Our work is not just about completing tasks. It is about helping older people stay protected, stay well, stay connected and remain living independently in their own home for as long as possible.

We are looking for an experienced and caring Support at Home Care Partner / Case Manager to join our team.

This role will suit someone who has strong experience in aged care, home care, case management, community services or a similar role. You will need to be confident managing a caseload, working with families, completing care plans, following up services and making sure people are receiving the right support.

About the role

In this role you will:

- work directly with older people, families and carers
- complete assessments and help develop care plans
- coordinate services such as personal care, domestic support, social support and community connection
- monitor care and follow up when needs change
- support clients to have choice, dignity and respect
- work with clinic staff, allied health, service providers and community supports




- manage documentation in line with funding and compliance requirements
- help older people stay safe and supported at home

About you

To be successful in this role, you will have:

- experience in Home Care Packages, Support at Home, aged care case management or a similar role
- qualifications in aged care, health, community services or a related area
- valuable organisation and time management skills
- confidence using client management systems and Microsoft Office
- the ability to manage a caseload independently
- an understanding of aged care quality, safety and compliance
- a National Police Check and Working With Children Check, or willingness to obtain these

Experience as an Enrolled Nurse, Aboriginal Health Practitioner, aged care case manager, community services worker or similar will be highly regarded.

Experience working with Aboriginal and Torres Strait Islander Elders, families and communities will also be highly regarded.

$85,000 to $95,000 plus superannuation and salary sacrifice, depending on experience.
