This position is based at the Warracknabeal campus, conveniently located halfway between Melbourne and Adelaide. About the Role
We are seeking an Intake Clinician to join our Intake Team, providing intake support to consumers accessing Community Services across all Rural Northwest Health campuses, working 48 hours per fortnight. In this role, you will deliver a responsive and person-centred intake service, coordinate referrals and connecting consumers with the most appropriate services. This is an excellent prospect to make a meaningful difference by improving access to quality healthcare for rural communities.
Key Responsibilities
- Provide a responsive, person-centred intake service by assessing consumer needs, goals, and support requirements to ensure appropriate access to Community Services.
- Coordinate referrals and service pathways, connecting consumers with the most suitable healthcare and community supports.
- Conduct intake assessments, gather relevant information, and maintain accurate documentation while ensuring confidentiality and compliance with policies.
- Collaborate with multidisciplinary teams, healthcare providers, and community organisations to support coordinated and timely care.
- Advocate for consumers and contribute to improving access, service quality,
and healthcare outcomes for rural communities.
The successful candidate will have:
- Qualifications as an Allied Health Assistant, Nurse or Allied Health Professional
- Experience in the implementation of person-directed care within a community setting
- Excellent written and verbal communication skills includingcomputer skills
- Strong time management skills with the ability to prioritise tasks
- Strong interpersonal skills and demonstrated collaboration within an inter-disciplinary team
- Ability to plan and organise own workload to support the functions of the team
- Excellent attention to detail
- Commitment and willingness to participate in continued training and education related to the role
- Knowledge of Primary Care Principles and chronic disease management principles is advantageous
- Current flu vaccination evidence
- Current Working with Children Check and Police Clearance or willingness to obtain one
- Current driver’s licence
*Must provide evidence of immunisation history and/or serology results.
Pay range and compensation package
This position is paid in accordance with the Nurses and Midwives (Victorian Public Sector) Single Interest Employer Agreement 2024-2028 (depending on qualification and experience)
