Care Connect is seeking a Care Manager to join the Out of Hospital Care program in Bella Vista on a full time, 12-month parental leave cover. You will assess, plan and manage services that enable people to safely return home from hospital and promote independence and choice.
You’ll work to build referral networks, connect clients with local resources, and ensure service excellence across all stages of the client journey.
