We See You is a small NDIS provider, founded and led by a social worker with a deep commitment to person-centred, trauma-informed care. We are seeking a compassionate and highly organised Mental Health Support Worker / Entry-Level Support Coordinator to join our team as a casual subcontractor.
About the Role
This is a dual role that combines hands-on support work with support coordination tasks. There is an opportunity to progress into support coordination, managing your own caseload. Below are examples of tasks you may be required to complete.
Support work tasks include:
- Supporting daily living activities such as meal planning, grocery shopping, and meal preparation
- Encouraging routines recommended by allied health professionals
- Assisting participants to develop new skills and increase independence in decision-making
- Providing companionship, social interaction, and encouragement with goal-setting
- Transporting participants to appointments, activities, and social events
- Supporting participation in hobbies, classes, and volunteering opportunities
- Assisting with everyday administration such as Centrelink appointments, paying bills, or housing applications
- Respecting participant preferences, cultural needs, and dignity while encouraging choice and control
- Working collaboratively with participants, families, and other providers
- Maintaining accurate notes and records
Entry-level Support Coordination tasks include:
- Researching provider options in accordance with participants' preferences
- Liaising with providers
- Completing referrals
- Completing service agreements, consent forms, and other forms
- Maintaining accurate notes and records
- Assisting with admin tasks
Support work is delivered both in participants' homes and in the community.
Support Coordination tasks can be completed remotely.
Flexible schedule: approx 6-10 per week to start, with the potential to increase as demand grows
Weekend availability desirable , weekday availability essential.
We See You is a remote organisation with no fixed office. You'll work independently day to day, with ongoing support and guidance from the Director.
