Position: Care Partner - Community
Type: 12-month max term, full-time

We are seeking a dedicated and capableCare Partnerto play a pivotal role in delivering high-quality, person-centred community care. This role is ideal for an experienced aged care professional who is passionate about coordinating meaningful support, leading frontline teams, and ensuring services align with both client needs and compliance standards.
You will work closely with clients, families, Community Care Workers, and internal stakeholders to ensure services are responsive, culturally respectful, and aligned with the Aged Care Quality Standards. This is a role with strong operational impact, leadership responsibility, and the opportunity to make a genuine difference every day.
This appointment is offered on a maximum-term basis of up to 12 months, with a strong potential for extension subject to organisational requirements and performance.
Key Responsibilities
- Coordinate client intake, assessments, reassessments, budgets, and individualised care planning
- Lead, support, and develop Community Care Workers through induction, supervision, training, and performance conversations
- Ensure the delivery of safe, person-centred, and culturally responsive services
- Maintain accurate, confidential, and compliant client documentation and records
- Build positive, professional relationships with clients, families, nominees, and external stakeholders
- Manage risks, incidents, feedback, and complaints in line with organisational policy and regulatory requirements
- Support Continuous Quality Improvement initiatives across community services
- Ensure secure work practices, WHS compliance, and infection prevention standards
- Support business sustainability through timely billing, claiming, budgeting input, and consumables management
- Promote St Basil's Community Care services within ILUs, community networks, and relevant forums
About You
You are an organised and compassionate aged care professional with strong coordination and leadership capability. You enjoy working closely with people, balancing operational priorities with quality outcomes, and ensuring older people receive care that reflects dignity, respect, and choice.
You bring:
- Experience in community aged care coordination, case management, or assessment
- Strong leadership skills with the ability to guide, support, and motivate care workers
- Highly developed emotional intelligence and relationship-building capability
- Excellent organisational, time management, and documentation skills
- Clear, confident communication and the ability to tailor conversations to diverse audiences
- Sound judgement, problem-solving skills, and a client-centred approach
- A solid understanding of the Aged Care Quality Standards and compliance requirements
- 12 months experience in a similar role is preferred
Mandatory Requirements
- NDIS Worker Screening Check
- Pre-employment medical clearance
- Right to Work in Australia
- Working with Children Check
- First Aid & CPR Certificate
- Current driver's licence and access to a fully insured vehicle (as required)
