Uniting Victoria and Tasmania – Ballarat VIC is seeking an Intensive Case Manager to deliver the Journey to Social Inclusion program. You will provide long-term, personalised support and case management to people experiencing chronic homelessness, using a trauma-informed, team-based approach to help clients access housing, build life skills, and foster independence.

The role is based in Ballarat in a full time, maximum-term capacity until 31 March 2028, with SCHADS Level 5 pay and
