About the opportunity

As part of HIP you will be based in a dynamic office working alongside a multidisciplinary team of Allied Health Staff and Specialist Nurses who provide a high level of support and care coordination to HIP clients within the Wyndham catchment. This is a high performing team working Monday to Friday, office hours. You are not required to work weekends or public holidays. HIP consists of 5 services: Community Based Rehabilitation, Complex Care (HARP), Falls and Balance Clinic, Pulmonary Rehabilitation and Continence Clinic.

The Senior Social Worker/Care Coordinator role services the Complex Care team as a Care Coordinator, and the other clinics in a Social Work capacity.

Supporting clients in the community and within the outpatient setting, your role would include:

- Conducting home visits, comprehensive psychosocial assessments, supporting clients to access services such as Centrelink, NDIS or VCAT, facilitating family meetings, completing appropriate referrals and presenting at case conferences.




- An extensive understanding and awareness of working in a community setting - community experience is desirable
- Demonstrated higher level of skills in clinical decision making, comprehensive psychosocial assessments, problem identification and analysis
- Extensive knowledge and experience in applying the most appropriate interventions to best support the client
- Being able to provide expertise in a specialty area to client and the HIP team, such as family violence, advance care planning, child protection, mental health assessment and support, disability support.
- Commitment and expertise in providing supervision to staff and students
- To actively participate in the development of care coordination /rehabilitation and social work knowledge and practice through quality assurance and research activities
- Utilising motivational interviewing techniques to help clients explore and resolve their uncertainty, as well as setting SMART goals

Skills and Experience

To thrive in this role, you will have:

- Excellent clinical, time management, organisational and communication skills, with the ability to adapt and problem solve.
- Broad range of experience in some of the following areas: advocacy, mental health, housing and homelessness, carer related needs, elder abuse or psycho social issues
- A compassionate, flexible and positive approach to effective patient care and management.
- The ability to nurture friendly, respectful relationships and contribute to a collaborative team environment.
- Demonstrated experience in providing a leadership role to promote teamwork and development





You will also be required to provide evidence of, or in the process of obtaining:

- A current Police Record Check and Working with Children Check.
- Proof of current influenza vaccination and other vaccination status consistent with public health orders.
- Full Victorian Driver’s License (no restrictions)
- Eligibility for membership with the Australian Association of Social worker
