ABOUT THE ROLE

As a Clinical Care Coordinator, you'll build lasting relationships with people living with complex mental health and psychosocial challenges and support them to navigate the health system, build on their strengths, and work towards their recovery goals.

DUTIES

- Undertake comprehensive and ongoing biopsychosocial assessments to identify, monitor and meet diverse individual needs
- Develop collaborative care plans in partnership with clients, families/carers, formal and informal support networks
- Provide evidence-based clinical care coordination, treatment and psychosocial support
- Provide person-centred, trauma-informed and culturally safe care
- Coordinate care with GPs, psychiatrists, allied health and community mental health services
- Provide assertive outreach to support transitions and reduce avoidable hospital presentations

SKILLS & EXPERIENCE

- Degree qualifications in a mental health profession (nursing, social work, or occupational therapy) with current AHPRA registration and/or relevant professional body accreditation
- Minimum 3 years' clinical experience




- Experience in community-based mental health service delivery, including care coordination and/or crisis interventions
- Ability to work autonomously and as part of a team within a strong clinical governance framework, and build effective stakeholder relationships
- Highly developed communication and interpersonal skills
- Current NSW Working with Children Check clearance
- Recent or new (employer facilitated) Criminal History Check. Please note that people with criminal records are not automatically barred from applying for this position. Each application will be considered on its merits.

This position is classified as Allied Health Professional 2 / Registered Nurse Level 1-2 under the Hunter Primary Care Agreement 2021.
