About The Role

The Wellbeing Mentor Lived Living Experience or Non-Lived Experience role provides assertive community outreach and coordination of service provision for people with significant mental illness funded through the Commonwealth Psychosocial Support (CPS) program. The goal is to promote independence and build confidence of individuals. As a Wellbeing Mentor, there will be opportunities for you to assist with working alongside participants, providing strengths-based, trauma-informed and recovery-oriented psychosocial rehabilitation services through your lived experience lens.

We have two Wellbeing Mentor Lived Living Experience or Non-Lived Experience roles opportunities available. The role entails:

- Providing case management and assertive outreach to support a portfolio of participants and their families in their own home and community
- Providing short term assistance to enable participants to work towards their goals
- Assisting to build capacity and working towards outcomes identified by the participant




- Developing and maintaining professional relationships with participants and stakeholders
- Providing accurate and timely case notes including risk management and recovery plans
- This role involves significant outreach covering a broad catchment area of over 10 LGAs in the South East region of Melbourne. Use of a personal vehicle will be required.
- Hybrid working model with flexible service delivery between 8am-8pm Monday-Friday based on consumer needs

It is a requirement of this position to meet all compliance requirements concerning NDIS Work Screening, Working With Children's Check, work rights and possess a current & valid driver’s licence.

What we are looking for

Do you hold formal tertiary qualifications in the community sector – Social Work, Psychology, Cert V in Mental Health or Mental Health Peer Work or other relevant studies? Do you have a robust understanding of community mental health and services in the South East region of Melbourne, and experience in case management? Do you have the passion to use your experience in supporting individuals requiring psychosocial support to create linkages within their community and build their capacity and aide in their recovery?

- Thrives in a fast-paced environment with a high workload and enjoys staying productive and engaged
- Ability to work both autonomously, and contribute positively within a team, sharing ideas and working collaboratively
- Effective listening skills & an adaptable communication style
- Ability to advocate for others and to identify appropriate support networks with your knowledge of local area services




- Adaptable to change and the ability to think on your feet
- Lived or living experience of mental ill-health, or as a carer for someone living with dual diagnosis/disability are encouraged to apply
- Qualifications in Intentional Peer Support or a willingness to undertake training
