Permalink Support Services is a trusted provider of disability and community support services, committed to empowering people with disability to live independently, participate actively in the community, and achieve their personal goals. We are seeking a compassionate and motivated Community Worker to join our team and deliver person-centred support while connecting clients with community services, programs, and resources that promote inclusion, wellbeing, and independence.

Key Responsibilities include but not limited to:
• Assess clients' individual circumstances, conduct client interviews, identify support needs and personal goals, and develop, implement and review person-centred support plans.
• Assess clients' educational, training, employment, housing, health and social support needs, and facilitate access to appropriate services and programs that promote independence and social inclusion.
• Monitor clients' progress, evaluate support outcomes, prepare case notes and reports, and recommend adjustments to support plans where required.
• Refer clients to government agencies, community organisations, allied health professionals and other service providers that can provide additional assistance and specialised support.
• Assess community needs,



available resources and service gaps relating to health, welfare, housing, employment, education and training, and connect clients with appropriate community resources and external support providers.
• Liaise with families, carers, support coordinators, community groups, welfare agencies, government bodies, private businesses and other stakeholders to coordinate services, address community issues and promote awareness of available community resources.
• Facilitate community participation by encouraging clients' involvement in recreational, educational, vocational and social activities that enhance wellbeing, inclusion and quality of life.
• Provide practical guidance, advocacy and capacity-building support to assist clients, including young people, families and people with disability, to overcome social, emotional, behavioural, financial and everyday challenges while achieving their individual goals.
• Support families and carers by providing information, education and coordination of services to strengthen family wellbeing and community participation.
• Prepare reports, submissions and documentation required for funding, resources, service delivery and government or organisational reporting, while maintaining confidential client records in accordance with organisational policies, NDIS Practice Standards and relevant legislation.




• Identify opportunities for community development initiatives and collaborate with community stakeholders to improve access to services and promote collective solutions to community needs.
• Ensure all services are delivered in accordance with organisational policies, NDIS Practice Standards, relevant legislation and continuous quality improvement requirements.

Qualifications:
• Relevant qualification in Community Services, Disability, Individual Support, Social Work, Welfare, or a related field.
• Minimum 1 year of relevant experience in community services, disability support, case management, or a similar role.

Skills & Other Requirements:
• Sound understanding of person-centred practice, community development principles, and client’s advocacy.
• Knowledge of the NDIS, community support services, and relevant Australian legislation.
• Excellent communication, interpersonal, and relationship-building skills.
• Strong organisational, case management, and problem-solving abilities.
• Ability to prepare accurate documentation and maintain confidential client records.
• Current NDIS Worker Screening Check, First Aid and CPR certification, Working with Children Check (where applicable), and a current Australian driver's licence.
