At Banksia Palliative Care Service (Banksia), our vision is that every person accesses and receives exceptional, personalised palliative care support at the right time and in the right place. We are seeking a compassionate and skilled Occupational Therapist to join our multidisciplinary community palliative care team and support clients to achieve comfort, dignity, independence and quality of life within their own homes.

About the role

As the sole Community Palliative Care Occupational Therapist, you will provide specialist occupational therapy assessment, intervention, care coordination and equipment management services for people living with a life-limiting illness.

Working closely with an experienced Allied Health Assistant and within a highly collaborative interdisciplinary team, you will support clients, families and carers through person-centred care that promotes comfort, safety and quality of life. You will also play a key role in equipment prescription, clinical consultation, staff support and continuous improvement initiatives.

This is a part-time position, working three days per week (0.6 EFT).

Key responsibilities

- Conduct comprehensive home-based occupational therapy assessments.
- Develop and implement client-centred intervention plans.
- Provide specialist advice regarding equipment prescription, installation and protected use.
- Coordinate equipment ordering and monitor delivery and follow-up processes.
- SWEP applications.
- Support clients, carers and families to identify goals and optimise function and comfort.
- Participate in interdisciplinary case reviews and care planning.
- Provide education,



coaching and consultation to nursing staff regarding equipment and functional strategies.
- Liaise with hospitals, community services and external providers to facilitate seamless care transitions.
- Maintain accurate clinical documentation and reports.
- Contribute to quality improvement, service development and organisational initiatives.

About you You are a highly organised and compassionate Occupational Therapist who believes in holistic care, seeks to make the lives of people living with life-limiting illnesses better and thrives working autonomously while remaining an engaged member of a multidisciplinary team.

Essential qualifications and experience

- Bachelor of Applied Science (Occupational Therapy) or Bachelor of Occupational Therapy.
- Current AHPRA registration.
- Eligible for membership with Occupational Therapy Australia.
- SWEP registration.
- Demonstrated ability to work independently and manage a complex workload.
- Strong interpersonal, communication and documentation skills.
- Current driver's licence and willingness to travel within the community.

Highly regarded

- Minimum two years post-graduate experience.
- Experience in community-based or inpatient palliative care.
- Experience working within an interdisciplinary healthcare team.
