About the Role

The Regional Health Chronic Disease Navigator (RHN) is a strategic position within Primary Care Connect (PCC) responsible for enhancing integrated, client-centred care coordination across the Goulburn Valley region. Based in Shepparton and servicing the Local Government Areas of Strathbogie, Greater Shepparton, Mitchell, Murrindindi and Moira, the RHN supports individuals living with chronic disease and complex health and psychosocial needs to access timely, coordinated and appropriate health and community services. Working within PCC's Chronic Condition program, the RHN facilitates seamless navigation across health and social care systems. The role strengthens regional service integration, improves care continuity, promotes shared care planning, and addresses barriers to access for vulnerable and rural populations. The RHN plays a critical role in building collaborative partnerships, developing streamlined referral pathways, coordinating multidisciplinary responses, advocating for clients, and contributing to continuous service improvement to achieve improved health and wellbeing outcomes for the community.

Position Context





This position reports to the Manager of Healthy Communities at Primary Care Connect and works within the Health Promotion Team. It is also expected that this position works within the larger Primary Care Connect teams, and networks with external organisations.

Key Selection Criteria

- Registered Nurse with AHPRA credentials
- Demonstrated experience in chronic disease management, care coordination, case management, health navigation or integrated service delivery.
- Strong understanding of the health, community, primary care and social service sectors, including referral pathways and service systems within rural and regional communities.
- Proven ability to establish and maintain effective partnerships with a broad range of stakeholders including health services, general practice, community organisations and government agencies.
- Highly developed communication, negotiation, advocacy and interpersonal skills, with the ability to engage effectively with clients, carers and service providers.
- Demonstrated ability to work independently while managing competing priorities, complex caseloads and service demands.
- Experience facilitating multidisciplinary collaboration, shared care planning and case conferencing.
- Robust organisational skills, including accurate documentation, data management and report writing.
- Demonstrated commitment to person-centred practice, continuous quality improvement and achieving positive client outcomes.
- Experience working within rural and regional health settings.
- Knowledge of chronic disease prevention,



management and self-management support frameworks.
- Experience using electronic client management systems and eReferral platforms
- Understanding of health system integration, population health approaches and service system reform initiatives.
- Experience working with vulnerable, socially isolated or culturally diverse populations.
