Southern Peninsula Community Support Inc is seeking a dedicated and experienced person who takes a person-centred and strengths-based approach to working with people. This is a new position that will lead a small but very experienced team providing short to medium term case management for people who are experiencing multiple and complex issues, with the aim of building capacity and empowering people to take action that is right for them.
Our current case management programs are the Low-Income Support Service (0.4 FTE) focused on couples and individuals while our Family Support Service (0.6 FTE) focuses on families with children under the age of 18. The Team Leader will play a leading role in developing our case management capacity over time. The position will have its own case load providing at least 0.5 FTE of their time conducting direct case management, primarily in the Low-Income Support Service.

We are the largest provider of Emergency Relief on the Southern Mornington Peninsula. We are a CISVic (Community Information & Support Victoria) member agency. CISVic agencies move beyond basic emergency relief using a wrap-around support model that provides a multi-layered support framework including generalist casework and homelessness support. The goal is to provide the best possible immediate material aid while connecting people with long-term, sustainable solutions.
SPCS is staffed by a team of committed and skilled volunteers, anda small team ofpaidworkers. Our case management programs,
the Low-Income Support Service and the Family Support Service are crucial to providing support at the complex end of that CISVic framework.
Key responsibilities
Lead a team of case managers to work with people who are in immediate crisis, experiencing financial stress or have multiple or complex needs to provide short to medium term case management and support through a combination of tailored information, options exploration, supported referral, advocacy and casework.
Oversee the delivery of case management services, ensuring quality standards and compliance with organisational policies and statutory requirements
Contribute to the development, implementation and review of the Low-Income Support Service and Family Support Service planning and operational processes.
Monitor caseloads across the team, ensuring appropriate distribution and timely progress on client outcomes
Develop and maintain effective relationships with community agencies, external services and partner organisations
Contribute to continuous improvement initiatives, identifying areas for program enhancement and best practice implementation
Represent the case management function in team and organisational meetings
What we’re looking for
Demonstrated team leadership,
understanding and experience in providing support and crisis intervention to individuals with complex needs in a case management model.
Knowledge of Mornington Peninsula welfare programs, the understanding of case management within an Emergency Relief service and knowledgeable of current evidence-based practice environment when supporting people with complex needs.
Strong knowledge of case management principles, assessment methodologies and care planning processes
Demonstrated ability to lead, motivate and develop staff in a compassionate and supportive manner
Excellent communication and interpersonal skills, with the ability to engage effectively with diverse stakeholders
Sound organisational and time management skills, with the ability to manage competing priorities and work independently
Proficiency and familiarity with case management or client management systems
