Your Health Place Medical Centre is a well-established general practice located in the heart of Glen Huntly. Our services extend beyond the clinic to include the provision of primary healthcare within Residential Aged Care Homes (RACHs) and other community settings.

Our Nurse Practitioner team is dedicated to supporting older adults by delivering comprehensive, evidence-based care that promotes health, independence, and quality of life. We proudly provide services across Southeast Melbourne and Castlemaine, working closely with residents, families, and healthcare professionals to achieve the best possible outcomes.

Position Overview:

We are seeking an experienced and enthusiastic Endorsed Nurse Practitioner to join our growing team. In this role, you will deliver advanced primary healthcare services to older adults living in Residential Aged Care Homes. You will be supported by an experienced Nurse Practitioner team, with access to ongoing mentorship and professional development within a collaborative and supportive environment.

Working both independently and alongside multidisciplinary healthcare teams, you will play a key role in health assessment, chronic disease management, preventative care, medication management, and health promotion. This position is ideal for a clinician who is passionate about improving the health and wellbeing of older people through high-quality, evidence-based care.

Essential skills,



experience and responsibilities:

Current registration as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA).

Endorsement by the Nursing and Midwifery Board of Australia as a Nurse Practitioner.

Demonstrated ability to prescribe and review medications, including deprescribing where clinically appropriate, in accordance with current legislation and best-practice guidelines.

Strong clinical assessment, diagnostic, and decision-making skills with the ability to work both autonomously and collaboratively.

Sound understanding of the legislative and regulatory requirements within the aged care sector.

Excellent interpersonal and communication skills, with the ability to build positive relationships with residents, families, general practitioners, and multidisciplinary healthcare teams.

The successful candidate will also hold:

Current CPR certification.

National Police Clearance.

Professional Indemnity Insurance.
