Northern Sydney Local Health District is seeking a Social Worker for the Community Aged Care and Rehabilitation Service (CAReS) at Ryde Hospital. The role supports older adults to live safely and independently at home, within a agile multidisciplinary team.

Temporary Full Time until 4 June 2027 with possible ongoing employment. The team participates in care planning, discharge planning, and quality improvement, with a redeveloped Ryde Hospital enhancing services and facilities.
