What you will do:
- Manage statutory processes for Education, Health, and Care Needs Assessments/Plans for an identified caseload of children and young people.
- Draft, develop, and amend Education, Health,
and Care Plans, ensuring they meet statutory requirements and reflect the needs and provisions agreed upon for children and young people.
- Facilitate and attend co-production and annual review meetings with families and professionals, encouraging effective collaboration and decision-making.
- Work in collaboration with parents, schools, and other agencies to develop effective relationships and ensure compliance with set agreements.
- Present cases to panels as necessary and provide feedback to ensure outcomes are actioned efficiently.
- Support monitoring of children and young people’s progress via the Statutory Annual Review process.
- Provide contribution to the planning and delivery of responses to special educational needs within the local authority.
