Salary: $83,734.08 - $123,940.80 pro-rata + Super + Salary Packaging

About the role
- Hospital in the Home (HITH) provides hospital-level care outside the traditional hospital setting, delivering short-term, high-intensity treatment through a multidisciplinary team to support patient recovery, improve outcomes, and reduce avoidable hospital admissions
- This is a part-time positionof 16 hoursper week, workedacross Thursdays andFridays,
with the opportunity to work additional hours to assist with sick leave and operational coverage when required
- Provide comprehensive pharmacyassessment, clinical interventionsand medication managementfor Hospital inthe Home (HITH) patients, including thefacilitation of ongoingintravenous medicationmanagement where required
- Work collaboratively withina multidisciplinary teamand with externalhealthcare providers, includingGPs, community pharmacists, community services andhospital providers, toensure coordinated, high-quality patient care
- Provide education andexpert advice topatients, staff andthe community onmedication management,promoting the QualityUse of Medicines, appropriate use ofmedication compliance aids, and client independence
- Contribute to serviceimprovement through qualityimprovement activities,clinical education, effectivecaseload management, andparticipation in teammeetings and policydevelopment
About you
- Current registration asa Pharmacist withthe Australian HealthPractitioner RegulationAgency (AHPRA), withno conditions onscope of practice, and a minimumof three years' post-registration hospitalpharmacy experience
- Demonstrated clinical knowledgeof medicines, includingantimicrobial stewardshipand Quality Useof Medicines (QUM) principles
- Demonstrated experiencein a similarrole, preferably withina community-basedor community healthsetting
- Demonstrated high-levelclinical assessment, criticalthinking, and problem-solving skills, withthe ability todeliver safe, high-quality, person-centredcare
