Northern Sydney Local Health District is seeking an Intake Nurse Clinical Nurse Specialist Grade 2 (CNS2) in the Hospital in the Home team. You will receive, process and triage referrals, assess patients, determine suitable care pathways and coordinate admissions to HITH services.

Working with medical, nursing and allied teams, you will perform rapid intake assessments, make autonomous clinical decisions, and ensure timely access to care in a collaborative, supportive workplace.
