MCWA is a in-home aged care provider with 40 years experience that focuses on supporting the multicultural ageing and elderly communities in Australia. We are currently seeking compassionate and dedicated Care Partnerto join our team based in the Melbourne, Australia.

About the role

Exciting opportunity for a person who is passionate about aged care, and supporting the ageing and elderly. The successful candidate will have a strong client service focus with demonstrated ability to maintain strong ethics and care.

Responsibilities

- Demonstrated ability to build strong partnership relationships with client
- Provide comprehensive case management for each client




- Build robust Care Plans that are appropriate for the individual needs of each client whilst meeting the limitations of each client’s budget
- Re-assess client care plans, goals, OHS assessment, and emergency plans for clients
- Maintain strong client notes
- Nimble in their ability to support clients

About You

- Ability to speak Greek language is required
- Proficiency in computer skills
- Excellent communication and interpersonal skills
- Ability to work independently and as a part of a multidisciplinary team
