Our Care Partners are more than case managers—they're trusted advisors who help older Australians navigate the recent Support at Home program & remain independent in their own homes.

Job description
As a Care Partner,



you will be the main point of contact for our clients receiving the Support at Home program. You will have the opportunity to work closely with clients, identify care needs, and facilitate the necessary services to help older people stay at home. The role includes care planning, advocating for clients where the need for reassessment is identified, and assisting clients to understand and manage their funding.

This role is an opportunity to provide meaningful support to many older Australians in need of care.

Required

- AHPRA Registration as a Registered Nurse (RN)
- National Criminal History Check / Police Check showing no disclosable Court Outcomes or Convictions, must be less than 3 years old
