About the role:
We have Permanent Part Time Night Shift (2145 - 0715) rosters available
- Roster 1: Wk 1 & 2 – Thu to Sat – 2145 to 0715 (27hrs/wk)
- Roster 2: Wk 1 & 2 – Sun to Wed – 2145 to 0715 (36hrs/wk)
Provide dedicated leadership and supervision to Personal Care Workers and support Registered Nurses in their role to promote a harmonious team approach to the delivery of resident care at Uniting AgeWell – Hawthorn
- Deliver high quality, person centered care to residents and families, including consideration for cultural, spiritual and individual needs
- Provide excellence in nursing care to residents with complex needs such palliative care,
dementia support, restorative care and pain management to enhance their wellbeing
- To support nursing staff regarding assessment and care planning processes, documentation and evaluation of care outcomesAnalyse risk, implement strategies and participate in clinical handover with Registered Nurses
- Promote a harmonious team approach by acting as a role model in coordinating daily routines and adhering to safe practice measures in medication management in consultation with the Registered Nurse
About you:
You are passionate about what you do and want to make a difference in the lives of our residents to support them to live life to their potential!
- Current registration with AHPRA as a Medication Endorsed Enrolled Nurse (Div 2)
- Minimum 2 years experience as an Enrolled Nurse in residential aged care (must have)
- Demonstrated experience with dementia care and palliative care support (desired)
- Excellence in decision making, critical analysis and on the spot problem solving
- Demonstrated abilities to delegate and prioritise in a fast paced workplace with exemplary communication skills in a team and one on one interactions with all levels of care partners
- Available and committed to work in the above mentioned roster vacancies at Hawthorn -unsuitable for students with inconsistent availability
- Thrive in a team and also appreciate the one to one interactions

Essentials for working in Aged Care:
- A current Working With Children Check employee card
- Current and ongoing flu vaccinations
- Ability to pass a national police clearance
- Hold a valid NDIS Worker Screening Clearance
