Responsibilities
- Be the initial contact for an older person and their support people, and using your patience and clinical knowledge via telehealth (phone/video conferencing) perform an initial needs assessment as part of the triage process.
- Using your decision‑making skills and clinical judgement determine the My Aged Care referral assessment type, eligibility status and priority/urgency for care.
- Act as a point of clinical escalation for Home Support assessors and complete conversions as required.
- Act as an authorised delegate for triage decisions, ensuring full compliance with single assessment contract rules.
- Identify and upscale high‑risk or complex cases to Senior Clinical Lead as per established work instructions.
- Conduct risk assessments for community assessors.
About the Candidate
- Be a qualified physio, OT, social worker or RN.
- Hold a current unrestricted registration with AHPRA or be eligible for a relevant professional association (e.g. Registered Nurse, Physio,
Occupational Therapist or Social Worker).
- Have at least one year of experience working in aged care or with older people.
- Authentic, fun, genuine, brilliant, approachable and a little bit cheeky.
- Knowledge of the My Aged Care pathway system is desirable.
- Demonstrate confidence when handling phone calls, building rapport quickly to complete triage calls efficiently and within expected timeframes.
- Be an empathetic listener who has a heart for calmly and respectfully engaging with clients, families, referrers and other stakeholders.
- Harness defined tools, technology and process which require specialised training.
- Thrive on maintaining concise and clear documentation, with robust keyboard skills and the ability to accurately capture information in real time.
- Be confident in using computer systems while engaging in phone conversations with ease and enthusiasm.
- Hold a valid full unrestricted driver’s licence.
