MND Keeping Connected Advisor .6 FTE 2year Fixed term Role- based in Canterbury, Victoria / remote working considered.

Join a passionate and dedicated team making a meaningful difference in the lives of people living with Motor Neurone Disease (MND) across Victoria and Tasmania.

We are seeking an experienced, compassionate health professional to join our team of 26 MND Advisors and Support Coordinators. In this rewarding role, you will support clients aged 65 and over through our Keeping Connected model - a specialised telehealth service designed for people living with MND who have a slower disease progression or a secure support network and home environment.

As a Keeping Connected Advisor, you will build trusted relationships with individuals and families impacted by MND, providing expert guidance and support throughout their journey.

You will play a key role in:

- Providing advice, education, and emotional support to help clients and families understand MND and its impacts.
- Undertaking timely and holistic assessments to identify the needs of clients.
- Connecting individuals with the most appropriate services and supports,



both immediately and in planning for the future.
- Coordinating support services and assisting clients to navigate aged care and palliative care systems.

The successful applicant will have:

- experience in working with people living with a progressive degenerative neurological disease
- excellent knowledge of support and care planning, assessment and referral in the, health, disability, palliative care and aged care service sectors
- a can do approach with a demonstrated client focused approach to all work activities
- A relevant tertiary qualification in a health/welfare related discipline

This is a truly rewarding position that offers the ability to specialise in working with people impacted by Motor Neurone Disease (MND)

While specific knowledge of Motor Neurone Disease is not essential, we do require relevant experience in neurological or progressive disease care. Extensive training and support will be provided to help you build your expertise in MND.

Excellent salary range $115,892.40-$121,141.64 pro-rata (plus super) and salary packaging available.
