Western Health is seeking experienced Registered Nurses for Paediatric Hospital in the Home (HITH) Liaison/Nurse roles at Sunshine Hospital. These part time, ongoing positions combine liaison, coordination and direct clinical care to support children and families across the care continuum.
As Paediatric HITH Liaison Nurse you will identify referrals, assess patients, provide clinical advice and telehealth support, coordinating safe transitions into HITH.
