About the Role

We are seeking expressions of interest from experienced Registered Nurses for a Paediatric Hospital in the Home (HITH) Liaison/HITH Nurse (dual role) based at Sunshine Hospital.

Our HITH service continues to grow, with exciting developments including the introduction of a dedicated Paediatric HITH stream. This dual/interchangeable position offers a unique opportunity to combine liaison, coordination and direct clinical care, supporting children and their families across the continuum of acute care.

As the Paediatric HITH Liaison Nurse , you will identify and coordinate suitable referrals, undertake patient assessments, provide clinical advice and telehealth support, coordinate care and ensure safe, seamless transitions into the HITH service.

As the Paediatric HITH Nurse , you will deliver inpatient‑equivalent care in the home through home visits and community outreach. This includes comprehensive clinical assessment, ongoing monitoring, administration of treatments such as intravenous therapies, patient and family education, supporting children to receive safe, high‑quality care in the comfort of their own home.

You may also work intermittently within the inpatient paediatric ward to support operational requirements, maintain clinical capability and strengthen partnerships.

Key Responsibilities

- Triage referrals and assess suitability for Paediatric HITH.
- Monitor daily workflow and coordinate patient admissions to ensure safe, timely access to the service.
- Perform comprehensive clinical assessments and escalate concerns appropriately.
- Deliver safe, evidence‑based nursing care within scope of practice across the home and hospital setting.

About You

- Minimum of 5 years' experience in an acute paediatric setting, with advanced clinical expertise.
- Highly desirable – Postgraduate qualification in Paediatric Nursing (or currently working towards).
- Current Victorian Driver's Licence.
- Demonstrated ability to work autonomously while building effective collaborative relationships within a multidisciplinary team.
- Strong leadership, communication and clinical decision‑making skills.
- Desirable – Previous experience working in a Hospital in the Home (HITH) service.
