The Monash Adult Mental Health Program Hospital Outreach Post-suicidal Engagement (HOPE) initiative provides psycho-social support for people who are identified as having presented with a suicidal or self-harm attempt, or suicidal or intentional self-harm ideation. The initiative aims to provide follow-up care and support for people who traditionally would not have received such intervention immediately after hospital discharge, with broader referral pathways and tailored psychosocial support.



The aim of the initiative is to prevent avoidable suicide attempts and reduce the number of completed suicides by providing targeted psychosocial support services to a cohort of people in an assertive manner in the community for up to 3 months from the time of discharge.

Provide community based support that enables consumers to build self-resilience and capacity to self- manage social, economic and environmental factors that contributes to suicidality.

About the Clayton HOPE Team

The Clayton HOPE (Hospital Outreach Post-suicidal Engagement) program is a mental health initiative by Monash Health designed to provide intensive support for individuals after a suicidal crisis or self-harm event to reduce the risk of repeat attempts. This Victoria-wide program, which originated at Monash Health in Clayton, offers up to 12 weeks of support through a multidisciplinary team, including psychosocial support workers, mental health clinicians, and peer workers with lived experience. Services include developing personalised safety plans,



psychological support to build coping skills, and connectingindividuals and their families with ongoing community resources.

About the role

Clayton HOPE employs an assertive outreach model designed to connect with clients who struggle to engage with conventional services and who require a more flexible approach to care.

The psychosocial support worker provides tailored assertive outreach to support the individual to recover from a crisis.
