Job Description

As the Survivorship & Integrated Care Manager, you'll provide clinical leadership to design, implement and evaluate a new blood cancer survivorship care program. You'll establish the service model and lead a pilot in partnership with consumers, healthcare professionals and health services, with a strong focus on improving access for people living in regional areas.

This is a Monday to Friday role with flexible hours, no nights, weekends or on-call, and the freedom to plan your week across office-based work, virtual collaboration, and stakeholder engagement activities. You’llalso be supported with onsite training and ongoing CPD to maintain and grow your professional practice.

A few extra things to know

- Contract role until mid-September 2027
- 0.8 FTE, reporting to the Head of Clinical Operations, with no direct reports initially
- Some domestic travel will be required to build relationships with regional and rural stakeholders

What you'll be doing

- Lead the design, implementation and evaluation of a best-practice national survivorship model, including governance, clinical guidelines and quality/outcome monitoring
- Define workforce needs and lead the team delivering survivorship services, ensuring alignment with the Leukaemia Foundation Model of Care and accreditation standards
- Build strategic partnerships and referral pathways with cancer centres, primary care and regional health services, and co-design the program with lived experience consumers




- Integrate survivorship into broader supportive care pathways and contribute to funding proposals and business cases for program sustainability and expansion

What we are looking for

- Current AHPRA registration (e.g. Nursing, Psychology, OT, Physiotherapy) or Social Worker with AASW membership, plus a Working with Children Check
- Experience in haematology/oncology and a strong understanding of survivorship frameworks and supportive care models
- Demonstrated leadership capability, including experience in people leadership, governance and stakeholder engagement, together with the ability to operate with a high degree of autonomy
- Excellent communication and relationship-building skills with the ability to collaborate effectively and develop strong partnerships with internal and external stakeholders
- Commitment to continuous career development and improving client outcomes

It's a bonus if you have

- Experience developing new models of care, programs or services
- Understanding of health system navigation in regional/rural settings
- Post-graduate qualifications in cancer, project management or a related field
