Senior Clinical Care Partner – Support at Home

We are seeking an experienced Registered Nurse to lead clinical excellence in our Support at Home program, mentoring staff and ensuring high‑quality, person‑centred care for older Australians living independently at home.

Role Overview

Report to the Integrated Community Manager and manage clinical governance, quality improvement and service coordination across the Support at Home program. Based at the Kippa Ring office, you will work closely with Care Partners, Allied Health professionals and multidisciplinary teams, providing oversight, coaching and direct client‑care support when required.

Key Responsibilities

- Provide clinical leadership across the Support at Home program
- Coach, mentor and support Care Partners and clinical team members
- Oversee complex assessments, care planning and service coordination
- Ensure care aligns with client goals, funding agreements and legislative requirements
- Drive clinical governance, quality improvement and compliance initiatives
- Build strong relationships with clients, families and external healthcare professionals
- Support service growth and deliver exceptional client outcomes

Desired Skills & Experience





- Current AHPRA registration as a Registered Nurse
- Robust knowledge of the Support at Home program and aged care reforms
- Experience coordinating complex community‑based clinical care
- Demonstrated leadership, coaching and mentoring experience
- Excellent communication and stakeholder engagement skills
- Strong clinical judgement and problem‑solving abilities
- Exceptional organisational and documentation skills
- A passion for person‑centred care and continuous improvement
