North Richmond Community Health (NRCH) seeks a passionate Complex Care Coordinator to join the AOD Outreach and Community Engagement Program. You’ll provide trauma-informed care coordination and assertive outreach, supporting people who use drugs to achieve their own goals within a multidisciplinary team.

The role involves holistic assessments, care planning, and referrals, working across a seven-day roster with evenings and weekends as required.
