Westcare Family Medical Centre are seeking an experienced, AHPRA-registered Registered Nurse (RN) or Endorsed Enrolled Nurse (EN). You will work closely with GPs and administrative teams to manage our treatment room, drive chronic disease management, and provide high-quality, patient-centred clinical care.

Key Responsibilities:

- Chronic Disease Management: Independently coordinate and prepare GPMPs (General Practice Management Plans) and TCAs (Team Care Arrangements)
- Health Assessments: Conduct comprehensive patient health assessments, including 75+ health assessments and 45-49 year old health checks
- Treatment Room: Perform wound care, immunizations, ECGs, and assist with minor surgical procedures
- Clinic Operations: Confidently navigate clinical software (Best Practice preferred)




- Minimum of 2 years' experience working as a Practice Nurse in a GP setting
- Current, unrestricted AHPRA Registration (RN or EN)
- High level of competence in care plans and health assessments
- Accredited Nurse Immuniser qualification, would be highly regarded
