Role Description This contract remote Support Coordinator role is responsible for working alongside NDIS participants to help them understand and implement their NDIS plans. Day-to-day tasks include coordinating services and supports, managing referrals to providers, and maintaining regular contact with participants to review progress and address emerging needs. The role involves case management activities such as preparing documentation, monitoring service delivery, and assisting with problem-solving when issues arise.

The Support

Coordinator will also collaborate with families, carers, and service providers, ensure accurate record-keeping,



and support participants to build independence and confidence in navigating the NDIS.

Qualifications

- Candidates should possess solid Interpersonal Skills and Communication abilities to build trust, engage participants, and collaborate with families and providers.
- Candidates should possess Case Management and Customer Service skills to coordinate supports, manage participant needs, and respond effectively to inquiries and concerns.
- Candidates should possess Technical Support skills and basic digital literacy to assist participants with online platforms, remote communication tools, and NDIS-related systems.
- Relevant experience in disability services, community services, social work, or a related field is beneficial.
- A qualification in social work, community services, psychology, or a similar discipline is advantageous.
- Ability to work independently in a remote setting, manage time effectively, and maintain accurate documentation is essential.
- Knowledge of the NDIS and person-centered practice, along with a commitment to inclusive, non-discriminatory support, is highly valued.
