About the role

As a Care Partner, you'll be the trusted point of contact for a portfolio of participants and their families — guiding them through their Support At Home Program Package, coordinating services and supports, and using your clinical understanding to make sure care plans truly reflect each participant's health, goals, and changing needs.

Day to day, you'll:

- Build genuine, ongoing relationships with participants, families, and carers

- Use your clinical knowledge to identify changing needs, risks, and opportunities to improve participant wellbeing

- Monitor participant wellbeing and budgets, and adjust supports as needs change

- Work closely with our geriatrician-led clinical team to elevate concerns early and get answers fast





- Keep accurate, timely records that support quality care and compliance

The practical stuff

- Full-time role based in our Burwood office

- Primarily office-based with some participant home visits

- Phone and laptop provided

- On-site parking available

- Professional development supported — training, mentoring from our clinical team

About you

- Clinical experience or a strong clinical understanding highly regarded — you might be a nurse, allied health professional, or an experienced care coordinator who knows their way around clinical needs, medications, and health deterioration

- You genuinely care about older people — and it shows in how you communicate

- Experience in aged care, home care, community services, or a similar participant-facing role

- Confident juggling multiple participants, priorities, and moving parts without dropping the ball

- Clear, warm communicator — on the phone, in writing, and face to face

- Comfortable with systems and technology (CRM, or care management platforms)
