$92,000 - $100,000 p.a. + 12% Super + Salary Packaging
At Aspire4Life , we empower communities with purpose and impact. This role is for a Clinical Aged Care Assessor based in Southeast Sydney, NSW , serving the Inner West and Southeast Sydney suburbs. It is a full‑time permanent contract, Monday to Friday with no weekend or shift work required.

Responsibilities
- Conduct holistic, client‑centred assessments independently within your region focusing on wellness and reablement.
- Capture client information and collaborate to develop support plans and unique goals.
- Support older Australians to maintain independent living by considering assistive technology, social support services, and community care.
- Utilize the integrated assessment tool with My Aged Care.
About you
- Experience in assessment and goal‑oriented support planning in aged, community, health or social services.
- Confident communicator with a positive approach when collaborating with clients, families,
staff and stakeholders.
- Local knowledge and ability to network with stakeholders within your region.
- Advanced IT skills with experience across multiple platforms (Microsoft Office Suite).
- Experience working with KPIs to ensure high‑quality client outcomes.
- Ability to manage your time effectively in an autonomous working environment.
What to bring
- Tertiary qualification as a Registered Nurse, Occupational Therapist, Physiotherapist with current AHPRA registration, or Social Worker with AASW registration.
- Minimum one year experience within aged care.
