Permanent Part Time (9 days per fortnight) Assessment and Facilitation Officer in the Child, Youth & Families department based in Morwell.
Key Responsibilities
- Complete holistic assessments of need to support the development of goal directed support plans promoting a strengths‑based approach in partnership with service providers.
- Conduct home‑based assessments for the HACC PYP in accordance with DFFH and Home and Community Care Program guidelines.
- Support younger people with disabilities or chronic health conditions to access government funded services or link into other government programs such as the NDIS.
- Ensure that individual client needs are met through collaboration and planning.
Qualifications & Experience
- Tertiary qualification in Social Sciences, Disability Studies, Allied Health professions or equivalent with demonstrated industry experience.
- Current First Aid and CPR Certificate Level 2.
- Valid Driver’s Licence in Victoria.
- Demonstrated ability to provide safe environments for children and young people and protect them from abuse and neglect.
- Knowledge of principles of community inclusion and participation with experience in delivering responses to community needs or service gaps.
- Experience in performing community and/or health assessments using a strengths‑based approach and developing associated care plans.
