About the role

We are seeking an experienced Clinical Case Manager (Registered Nurse) to join our team on a Part Time basis, supporting clients across Aged Care, Disability, and severe personal injury programs in the Melbourne region.

You will provide clinical nursing procedures, manage client intake and assessments, manage budgets, develop care plans, deliver case management, support and train care/support workers, and coordinate all aspects of service delivery to ensure clients’ needs are met.

About you

- AHPRA Registration as a Registered Nurse with experience in community-based nursing (preferred)
- Experience in supporting clients in aged care, NDIS, and/or insurance schemes (TAC, Worksafe, icare)
- Passion for patient-centred care grounded in high-quality clinical outcomes.
- Outstanding interpersonal, communication, and problem-solving skills.




- Ability to work within multi-disciplinary teams.
- Enjoy a mix of working remotely, visiting clients' homes and in the office.
- Valid Australian driver's license and access to a reliable car with comprehensive insurance.
- NDIS Worker Screening and Working With Children Check (or willing to obtain).

The salary package will include a car allowance and the opportunity to earn performance-based bonuses.
