About the role

We are urgently seeking flexible and energetic social workers, preferably with experience in palliative care and/or mental health, to work across our health care services both on the ward and in the community.

This casual Level 1‑2 role is for immediate start at Neringah Hospital (Wahroonga) on a part‑time basis until the end of August 2026. It is available 1‑3 days per week and must include work on Fridays. An optional fixed‑term 1‑day contract is also possible until the end of August, with additional casual days preferred later in the year across our locations in Northern Sydney.

Responsibilities

- Work within HammondCare’s Mission in Action to provide a high standard of comprehensive Social Work service to complex clients at various health care sites.
- Operate within integrated specialist multi‑disciplinary teams in Palliative Care (inpatient and community) and Older Persons Mental Health (inpatient only).
- Provide clinical and qualified social work supervision and support to carers and families.
- Assess, intervene, counsel, advocate and manage complex discharge planning in a fast‑paced environment.
- Maintain documentation, communication and policy requirements, including handover systems and patient Electronic Medical Records.
- Maintain knowledge of relevant community services and referral systems (e.g., MAC, NDIS, ACAS) and facilitate seamless transfer of clients.
- Participate as an effective member of the multi‑disciplinary team, contributing to comprehensive needs assessments, goal setting and client‑centred interventions.

Essential Criteria

- Recognised tertiary qualification in Social Work and eligibility for AASW membership; overseas qualifications must provide an AASW Overseas Assessment number.
- Current working rights in Australia (citizenship, permanent residency or appropriate visa).
- Commitment to engage with HammondCare’s Mission in Action.
- Knowledge, skills and experience in broad social work interventions including psychosocial assessment,



counselling, advocacy, service liaison, problem‑solving and complex discharge planning.
- Experience delivering comprehensive Social Work Services in a community health or hospital setting – preference for palliative care or mental health.
- Knowledge of community services, supports and referral systems such as MAC, NDIS, and ACAS.
- Effective interpersonal and communication skills across written, verbal and digital media.
- Self‑motivation and time‑management skills to prioritise competing commitments.
- Flexibility, empathy and adaptability.
- Ability to work independently and collaboratively within a multidisciplinary team.
- Commitment to ongoing professional development and supervision.
- Willingness to travel between HammondCare sites as required.

Desirable Criteria

- Current unencumbered driver's licence and safe‑driving record.
- Previous experience within the NSW Health system, including use of NSW Health Electronic Medical Record (EMR).
- Experience working in a palliative care context.
- Experience working in older persons mental health.
