NDIS Worker Screening Check number (We are aware the document still says Police Check; this has been recently changed) Evidence of attending the Educational Institution (EI) A letter confirming Student is: Currently Studying covered by EI's Insurance a copy of the Learning Plan/Workbook Applicant Is not listed on the Aged Care or NDIS banning registers.
