Upstream Social Worker

Full time Fixed-Term | Newman, WA

Puntukurnu Aboriginal Medical Service (PAMS) is seeking an experienced and passionate Upstream Social Worker to join our Upstream Health team. This role focuses on addressing the social determinants of health through case management, advocacy, community engagement, and early intervention support for individuals, families and communities across the East Pilbara.

About the Role

Reporting to the Director of Upstream Health, you will:
Provide case management, therapeutic support and advocacy services.
Support individuals and families to overcome barriers impacting health and wellbeing.
Work collaboratively with Community Health Workers, health services and partner agencies.
Drive community-led solutions that address social and structural challenges affecting health outcomes.
Contribute to service development, reporting and continuous improvement activities.

About You

You will have:




A bachelor’s or master’s degree in social work, Psychology, Mental Health or a related field.
Registration with the Australian Association of Social Workers (AASW).
Experience in community development, Aboriginal health, family support, social work or community services.
Solid stakeholder engagement, advocacy and communication skills.
A current C Class Driver’s Licence and willingness to travel to remote communities
