Complex Triage Team | Better Health Network (BHN)

At Better Health Network (BHN), we believe everyone should be able to access the right support at the right time.
As a Triage Clinician, you'll play a critical role in helping people with complex needs connect to the service that will have the greatest impact on their health and wellbeing.
Working within our Resilient Communities Program, you'll be the important first point of contact for clients, ensuring they receive timely, person-centred support and are connected to the most appropriate service from the outset.
About The Role
This is a dynamic and rewarding role for an experienced clinician who enjoys problem-solving, complex assessment and helping people navigate services.
You Will
Conduct triage and screening for clients with complex needs
Assess risk, needs and service suitability to determine the best pathway for care
Support first-call resolution and reduce wait times for clients
Coordinate referrals and appointments across a range of programs
Work closely with clinical teams, Service Access and Support, and external referrers
Help improve referral pathways, service design and intake processes
Contribute to training and capability-building across frontline teams
Services currently within scope include Mental Health, Counselling, Family Violence, Community Health Nursing, Community Connections and Indigenous Access programs.
About You
You're an experienced clinician who can quickly build rapport, assess complex situations and confidently navigate service systems.
You'll Bring
Qualifications in mental health, counselling, psychology, alcohol and other drugs (AOD), social work or a related discipline
Significant experience triaging clients with complex needs in a community setting
Robust assessment, risk identification and decision-making skills
A client-centred, strengths-based approach
Excellent relationship-building and stakeholder engagement skills
The ability to work independently while collaborating effectively across multidisciplinary teams
Resilience, adaptability and strong organisational skills
Highly regarded
Membership of a relevant professional association
Knowledge of community services across Melbourne's south-east

Location
Based primarily at our South Melbourne site, with occasional travel to other BHN locations as required.
