The role involves psychosocial assessment, discharge planning, education and referral across acute inpatients, outpatients and aged care residents.
Grade 2 includes supervision of students and advanced clinical responsibilities.
Full time 80 hours per fortnight (ADO) or negotiable part time, ongoing position under Victorian Public Health Sector enterprise agreement.
