Acute Care Transition Coordinator (ACTC)
Permanent, Full Time – Registered Psychiatric Nurse Grade 5 (NP51).
Works within the Acute Mental Health Inpatient Service (AIS).
Key Responsibilities
Coordinate bed management and bed flow to and from the AIS to maximise efficiency.
Support the organisation to achieve KPIs and government targets, including NEAT.
Assist students, graduates and early career nurses – provide support, mentoring, training and education.
Work collaboratively with multidisciplinary teams to optimise patient care and service delivery.
Qualifications & Experience
Degree or postgraduate diploma in psychiatric/mental health nursing.
Completed or working towards a higher degree in nursing or a relevant area.
Current registration with the Australian Health Practitioner Regulation Agency (AHPRA) Division1.
Minimum 5years post‐qualification experience within mental health services.
Additional Requirements
Required to undergo mandatory immunisations/vaccinations, including flu vaccinations.
Must meet Victorian government direction regarding vaccination against COVID‐19.
