Are you passionate about supporting Aboriginal and Torres Strait Islander Elders to access culturally appropriate aged care services?
Gidgee Healing is seeking a dedicated Elder Care Connector to join our Elder Care Support team in Mount Isa.
, As the Elder Care Connector, you will:
* Conduct community outreach to promote aged care services.
* Engage with Elders and families to raise awareness of aged care entitlements.
* Support clients in identifying their goals and accessing appropriate services.
* Assist with My Aged Care registration and review processes.
* Advocate for clients and help identify service gaps in the region.
* Work closely with the Aged Care Support Coordinator and internal teams.

About the Candidate
* Certificate in Community Services or relevant experience.
* Strong relationships with local Aboriginal and Torres Strait Islander communities.
* Excellent communication and interpersonal skills.
* Understanding of person-centred care and trauma-informed practice.
* Experience working with older people and knowledge of aged care services.
* Ability to work independently and as part of a team.
Additional Requirements
* Current QLD C Class Driver's Licence.
* Satisfactory police check and current Blue Card (or willingness to obtain).
* Flexibility to travel and stay overnight in remote areas, including travel by road and small plane.
