Support Worker – Transitional Care (Residential Setting)

We are currently seeking compassionate and capable Support Workers to join our Transitional Care team in a residential setting, supporting participants transitioning from acute hospital care into a protected and supportive home environment.

About the Role
Working under the guidance of Registered Nurses and the Clinical team, you will support participants with a range of daily living and lifestyle activities while helping deliver high-quality, evidence-based care.
Your responsibilities will include:
Supporting participants with personal care, mobility, hygiene, toileting and feeding
Assisting with medication and health-related support activities
Supporting social engagement, lifestyle goals and community access
Maintaining accurate documentation and shift records
Supporting participants during admissions and discharges
Contributing to care planning and participant goal outcomes
Household domestic duties to maintain a safe and welcoming environment
Working collaboratively within a multidisciplinary team
Participating in training, meetings and continuous improvement initiatives

Essential Requirements
Certificate III or IV in Individual Support, Disability, Aged Care or similar (preferred)
Experience in disability, aged care, transitional care or residential support
Current First Aid & CPR
Current NDIS Worker Screening ClearanceBlue Card (or willingness to obtain)
Valid Australian Driver's Licence
Strong documentation and computer skills
Availability across a rotating roster, including weekends (morning and afternoon shifts only, sleepover only required if we onboard complex clients)
