Young People Leaving Care Outreach Support Worker.
Covering Liverpool, full time, 37.5 hours a week on an ongoing temporary basis- this could last a long time and has no end date.Outreach – hours are between 9am and 5 pm, between Monday To FridayThis is a temporary positon on an ongoing basis, if you are subscribed to the update service and have a short notice period, a quick start is possible.
£16 -£17+ an hour plus mileage coveredYou will meet with Care Leavers aged ***** to check in and ensure they have all the provisions and support available to them ready for when they are 18.Salary when and if permanent is approx £27K depending on experience and level 3You will need to drive and have business use on your insurance allowing youto travel to different places of work.Mileage is paid at 55p per mile.This is a outstanding opportunity to join a very well established company that have supported young people for many years – a true market leader in this sector.

a quick start is possible if you have a current online DBSIt is essential that you have experience of working within a similar setting and preferably have completed NVQ3 Children & Young People Workforce or a similar qualification within this sector.Qualifications/ RequirementsHigh standard of general education.Preferable: NVQ Level 3 Health & Social Care – Children and young People (CCYP) or equivalent.Willingness to undertake further training.Job PurposeTo be responsible for the provision of Support for Vulnerable People (Service Users) for the Company.Main Duties & ResponsibilitiesTo assist the Young person's allocated Social Worker in the delivery of the service.Provide transport/escort to the young person as directed in line with care plansConduct welcome and induction for Young peopleOngoing assessments of Service Users' needsProvide ongoing support and monitoring of Service UsersAssist in welfare and NHS issuesPreparation of reportsArrange appointments with various partner agenciesMaintaining regular contact with Service UsersInforming Service Users of support groupsAssist Move-On of Service UsersTo accurately record, create, maintain and update electronic records in respect of our Service Users.To prepare for and attend meetings as required or directed.To provide mediation to Service Users as required and/or directed.To effectively use IT packages to support the work of the company.To work effectively and cooperatively across multi disciplinary teams.To observe and maintain strict confidentiality in respect of the Service Users, Clients and Company.To pro-actively assist in the development of policies and procedures of the Company.To report any housing issues to the Services Department and to assist, where required, in the resolution of any defects.To undertake On-Call duties as directed.To participate, where required, in office reception duties.
