Firstrate Care Services Pty Ltd is seeking a dedicated and experienced NDIS Service Coordinator to join our team in Ipswich, Queensland.
As a NDIS Service Coordinator, you will play a crucial role in ensuring the smooth delivery of high-quality care services to our valued clients.
In this full-time position, you will be responsible for coordinating and overseeing the care provided by our support workers, ensuring that our clients' needs are met with the utmost care and professionalism.
Here's a general overview of the position description:
Coordination of Services
Develop, implement, and monitor individual support plans based on participants' needs.
Liaise with participants, families, and service providers to ensure quality care.
Organize appointments, services, and support activities.
Participant Support
Assist participants in understanding their NDIS plan and how to access supports.
Advocate for participants to ensure their needs and preferences are met.
Provide guidance on choosing service providers and managing the participant's plan budget.
Compliance and Reporting
Ensure compliance with NDIS policies and standards.
Maintain accurate records and reports on participants' progress.
Report incidents and update risk management plans when necessary.
Team Collaboration
Work with support workers, therapists, and other care providers to ensure integrated care.
Participate in meetings with families and external stakeholders to review care plans.
Crisis Management
Provide support during emergencies and adjust care plans as needed.
Assist participants in problem‐solving and accessing emergency resources.
Key Responsibilities
1. Participant Engagement and Support
Initial Assessment and Planning
Conduct thorough assessments of participants' needs, abilities, and preferences in collaboration with families or guardians.
Develop a tailored NDIS plan focusing on the goals and aspirations of the participant.
Assist participants in setting realistic and achievable goals under their NDIS plan.
Support in Navigating the NDIS System
Educate participants and their families about the NDIS, how it works, and their rights under the scheme.
Guide participants in understanding their plan's funding, including core supports, capital supports, and capacity‐building options.
Help participants manage their NDIS budgets, ensuring that funds are allocated appropriately and in line with their goals.
Advocacy
Act as an advocate for participants,



ensuring their voice is heard in service planning and delivery.
Ensure participants have the tools to make informed decisions about the support and services they receive.
2. Coordination of Services
Service Provider Management
Coordinate and maintain staff rosters, ensuring appropriate coverage aligned with participant needs
Monitor and adjust rosters in response to operational requirements and staff availability
Participate in a rotating on‐call roster to provide after‐hours support and incident response
Assist participants in identifying, selecting, and engaging appropriate service providers that align with their needs and goals.
Coordinate and manage relationships with multiple service providers, ensuring seamless and holistic care across various domains (e.g., healthcare, therapy, personal care).
Negotiate services and terms with providers to ensure quality and value-for-money services.
Monitoring and Adjusting Support Plans
Regularly review and monitor the participant's NDIS plan and progress, ensuring the participant's needs are met.
Facilitate plan reviews, making necessary adjustments as circumstances, needs, or goals evolve.
Ensure flexibility in adapting services, especially when participants' situations change (e.g., health changes, moving locations).
Organize and schedule services, including transport, health appointments, therapy sessions, and social or community activities.
3. Crisis Intervention and Problem‐Solving
Crisis Management
Respond to emergencies or critical situations swiftly, ensuring participants receive timely and appropriate support.
Collaborate with healthcare providers, family members, or emergency services during crises to ensure participants' safety and well‐being.
Conflict Resolution
Mediate disputes or issues that may arise between participants and service providers or within support networks.
Implement strategies to resolve conflicts while protecting the participant's interests.
4. Stakeholder Engagement and Collaboration
Team Collaboration
Work closely with support workers, allied health professionals (e.g., occupational therapists, speech therapists),



and service providers to ensure comprehensive and consistent care.
Facilitate regular team meetings to align on care goals and assess the participant's progress.
Engage with community organizations, advocacy groups, and governmental bodies to access additional resources and support.
Family and Caregiver Liaison
Act as the main point of contact for families and caregivers, ensuring they are kept informed and involved in decisions.
Provide emotional support and guidance to families, helping them navigate the challenges of disability care.
5. Administration and Documentation
Record‐Keeping and Reporting
Maintain detailed and accurate records of participant interactions, care plans, service delivery, and outcomes.
Document all incidents, complaints, and changes in service in compliance with NDIS and organizational policies.
Prepare reports on participant progress, funding utilization, and plan reviews for both internal use and submission to the NDS.
Compliance and Risk Management
Ensure that all services provided meet the regulatory standards and compliance requirements of the NDIS.
Develop and update risk management strategies, ensuring participants receive safe and high‐quality care.
Monitor and address any issues related to service provider performance, participant safety, or potential policy violations.
6. Capacity Building and Independence
Support Development of Skills
Assist participants in developing skills to live independently, manage their own care, or engage more actively in the community.
Organize activities, workshops, or access to community resources that promote social inclusion, employment opportunities, or education.
Encouraging Participant Self‐Management
Empower participants to take control of their NDIS plan by teaching them how to manage supports independently over time.
Encourage participants to advocate for themselves when interacting with service providers or other stakeholders.
Learning Chance
Engage with and respect diverse cultural perspectives in practice.
Utilize social work knowledge and theoretical frameworks in support coordination.
Requirements
Strong knowledge of the NDIS and disability services.
Minimum 2 year of relevant experience
Excellent communication, advocacy, and interpersonal skills.
Ability to manage and prioritize tasks, problem‐solving skills.
Experience in case management or care coordination.
Relevant qualifications in community services, disability care, or similar field.
