United by our Christian purpose and values, we stand together with anyone who needs us, for as long as they need us.Case Manager – Key ResponsibilitiesDeliver comprehensive, person‐centred case management for individuals experiencing homelessness or at risk of homelessness with complex needs.Undertake client assessments, develop holistic case plans and review progress towards agreed goals.Coordinate referrals and connect clients with appropriate internal and external support services.Provide evidence‐based interventions that promote independence, wellbeing and positive outcomes.Support clients through transitions, helping them develop the skills and confidence to self‐manage and access ongoing supports.Build strong partnerships with community organisations, government agencies and service providers to enhance client outcomes.Facilitate group programs, education sessions or outreach activities where required.Maintain accurate case notes, client records and reporting in line with organisational and funding requirements.Contribute to continuous improvement initiatives while supporting a collaborative and inclusive team environment.Provide mentoring and guidance to colleagues where appropriate.Qualifications and ExperienceRelevant tertiary qualifications in Social Work, Community Services, Human Services or a related discipline,
