Silverchain are recruiting Registered Nurses to support a new chronic disease service across the Blacktown and Cumberland LGAs with Wentwest.
The role will focus on assessment, care planning and coordination for people living with complex chronic conditions, working closely with GPs, practice teams, and specialist services.About YouRelevant qualification in Nursing with demonstrated experience in field of expertiseExperience in delivering chronic condition management, health care navigation, health coaching and the principles of health literacyWell-developed communication (verbal, written and listening) and interpersonal skills to liaise with clients, carers, families, and other health care professionalsDemonstrated ability to provide education, support and coaching to clients and their carersDemonstrated time management and organisational skills to effectively plan and prioritise workDemonstrated ability to work both autonomously and with a team to participate and contribute to program and organisational objectivesA demonstrated empathetic and flexible approach in all interactions with clients, carers and other health providersAbility to work autonomously or with minimal direct supervisionBased on Silverchain Group Policy and Government Directions, you will be required to provide evidence of vaccination against COVID-19 including booster, or evidence of a medical exemption in order to commence employment with Silver Chain.Benefits of working at SilverchainWe offer hands on training program,
