This is an opportunity to join a values-led service at an important stage of clinical improvement and Support at Home development.About the OpportunityOur Client is seeking an experienced Home Care clinical leader to oversee clinical governance, nursing services and complex care delivery across its community care program.
Working closely with the Home Care Manager, you will lead the nursing team and progress an established clinical improvement action plan.
A major focus will be strengthening clinical systems, managing risk and preparing the service for ongoing success under Support at Home.You will provide leadership and professional support to Registered Nurses, Enrolled Nurses, care partners and support workers while ensuring clients receive protected, evidence-based and person-centred care.Key responsibilities will include:Leading clinical governance and continuous quality improvementProgressing clinical improvement and Support at Home readiness initiativesOverseeing comprehensive clinical assessments and complex care planningMonitoring clinical risks, incidents, trends and quality indicatorsLeading incident investigations, root cause analysis and corrective actionsSupporting wound care, medication management and chronic disease programsCoordinating services with GPs, allied health professionals and hospitalsMaintaining compliance with the strengthened Aged Care Quality StandardsProviding clinical oversight across DVA,
NDIS and icare-funded servicesCoaching and mentoring clinical and non-clinical team membersImproving clinical documentation, reporting and care monitoring systemsBuilding a positive culture of accountability, safety and open communicationAbout YouYou will be an experienced Registered Nurse with a strong understanding of clinical governance and service delivery within a Home Care or community care environment.You will be comfortable identifying gaps, implementing improvements and supporting teams through operational and regulatory change.
Importantly, you will know how to balance compliance requirements with practical, person-centred care.To be considered, you will require:Current unrestricted AHPRA registration as a Registered NurseBachelor's degree or postgraduate qualification in NursingAt least two years of Home Care management experienceAt least three years of post-registration nursing experienceExperience leading Registered Nurses and Enrolled NursesStrong clinical assessment and complex care planning capabilityKnowledge of Support at Home and aged care regulatory requirementsExperience managing clinical incidents, risks and improvement plansStrong stakeholder management and multidisciplinary coordination skillsConfidence coaching care staff and building clinical capabilityCurrent First Aid and CPR certificationCurrent or obtainable NDIS Worker Screening ClearanceExperience with DVA Community Nursing, NDIS high-intensity supports, icare, wound management, dementia care, medication safety or chronic disease management will be highly regarded.Why Consider This Role?
