/n This is a unique opportunity to play a critical leadership role supporting people with mental health, high-intensity, and complex clinical needs to rebuild independence and quality of life.

/n Melbourne North / North West - Port Melbourne, Footscray to Melton and surrounding suburbs - Need to travel to Ballarat one day per week

- Registered Nurse (AHPRA registered)
/n
- Behaviour Support Practitioner (BSP)
/n
- Allied Health Professional
/nIf you have experience supporting individuals with mental health, high-intensity supports, or complex behaviours, this role offers the opportunity to lead services that make a real difference. You will oversee safe, structured hospital-to-community transitions, ensuring individuals receive the right supports to thrive in community living.
Key Responsibilities/n
/n
- Lead a portfolio of clients with complex and high-intensity support needs
/n
- Manage hospital-to-community transitions, ensuring safe and sustainable care arrangements
/n
- Build, mentor and lead high-performing frontline support teams
/n
- Oversee care plans, behaviour support plans, and clinical risk management strategies
/n
- Collaborate with hospital discharge teams, allied health professionals, families, and stakeholders
/n
- Drive quality, compliance, and continuous service improvement
/n
- Balance clinical oversight with effective portfolio management
/n What You'll Bring/n
/n
- A background in Nursing, Behaviour Support, or Allied Health (preferred)
/n
- Strong leadership experience (minimum 2 years managing teams)
/n
- Experience supporting individuals with mental health and/or complex care needs
/n
- Strong knowledge of Positive Behaviour Support and risk management
/n
- Confidence working within multidisciplinary teams
/n
- A collaborative approach to working with families, clinicians and hospital teams
/n
- A passion for delivering high-quality, person-centred services
/n
- Experience within the NDIS and disability sector
