CANDIDATES RESIDENT IN CABOOLTURE QLD AREA AND SURROUNDS MOSTLY PREFERRED

We are seeking Disability Support Workers for our participants who requires Person-Centered support in Caboolture. This position would suit both experienced and newly qualified but motivated Support Workers looking for a casual shifts with a view to ongoing shifts to provide ongoing support to our participants. Flexibility in hours and ability to work active awake shifts and sleepovers highly preferred with guaranteed hours.

The position and requirements

We're seeking passionate Disability Support Workers to make a difference in the lives of people with disability. Your role will see you working closely with individuals, building connections and developing pathways to ensure they are able to achieve their individual goals and maintain socially valued roles, friendships and relationships.

Reporting to the manager, you will work alongside people with physical/ mental Health and intellectual disabilities, helping them to access opportunities by connecting them with appropriate community contacts. You will ensure that opportunities for work, membership to social groups and volunteering opportunities are accessible, encouraging and supporting individuals to achieve their goals while staying happy, healthy and protected.

This hands-on role will also require you to ensure the health and safety of our clients and team members are at the forefront of everything we do.

About the role





We’re looking for team members who can demonstrate previous experience supporting clients with disability with an ongoing and guaranteed hours for both weekdays and weekends and also have the ability to work active awake and sleepovers. Applicants living in Caboolture or nearby suburbs mostly preferred.

Key Responsibilities

- Skills development training for Secondary School Leavers, Follow individual support plans, behavior support plans and health care plans
- Participate in community activities
- Maintain and monitor a client's support plan whilst modelling a person-centered approach
- Identify and support a range of learning opportunities for our clients
- Observe and document an individual's progress and any changes through detailed note keeping

Core Responsibilities

- Personal care which includes dressing, toileting and showering
- Assist participants with Meal preparation, shopping, housekeeping and cleaning
- Assist participants in both taking and prompting medication with relevant training provided.
- Experience with hoisting or associated training provided.
- Supporting participants with incontinence issues including changing aid
- Assistance with client engagement with community and social activities
- Supporting clients to travel to and from activities and appointments as required




- Maintaining daily notes for client activities
- Completion of required reports and documentation
- Ongoing and positive communications with other client stakeholders, such as carers, family and staff.

Skills & Experience

- Minimum Qualification – Cert III in Individual Support
- Current QLD Drivers License
- Current First Aid Certificate & CPR
- Demonstrated commitment to supporting and engaging with low and high support clients
- Proven success managing and monitoring challenging behaviours
- Strong communication skills with the ability to build and maintain positive relationships.

Salary: starts at $33 per hour, could be increased based on KPI
